How to Request Classroom Accommodation for Religious Holiday

A student is permitted to be absent, without penalty, for up to three days each academic semester to take holidays for reasons of faith or religious or spiritual belief system or to participate in organized activities conducted under the auspices of a religious denomination church, or other religious or spiritual organization.

Following UToledo Policy 3364-71-30 Religious accommodations and Procedure 3364-71-30.01:

Step 1:   Inform instructor(s) as early as possible. 

Step 2:   Complete all required fields of the request form.

Note: A separate form must be completed for each course and all request forms must be submitted within 14 days of the start of instruction.

Step 3:   A memo will be emailed to your instructor(s) informing of them of your request and you will be copied on the email. 

Step 4:   Once the memo is sent, discuss with your instructor(s) plans to make up any work that you may miss.
